Title V Information System
Health Resources and Services Administration/Maternal and Child Health Bureau (HRSA/MCHB)
TVIS includes data on each State's needs, priorities, program activities, and performance and outcomes measures which can be compiled and compared with information from other States. A clinical exam was also performed.
Social Determinants of Health
United States, Jurisdictions and Territories
An application must be submitted to MCHB each year that contains a statewide needs assessment (to be conducted every 5 years) that will identify (consistent with the health status goals and national health objectives) the need for preventive and primary care services for pregnant women, mothers, and infants up to age 1; preventive and primary care services for children; services for CSHCN (children with special health care needs). The application and annual report are to be completed using the Title V Information System Electronic Reporting Package (ERP). The State must submit all narrative, charts, supporting documents, and exported ERP data files on one or more standard 3.5 inch floppy disks.
TVIS is designed to capture data contained in the States' block grant application/annual reports. It gathers data that would aid in extending and improving health and welfare services for mothers and children.

Compendium of Publicly Available Datasets and Other Data-Related Resources. The National Partnership for Action to End Health Disparities has compiled a Compendium of Publicly Available Datasets and Other Data-Related Resources (Compendium). This free resource details publicly available data relevant to research and programs aiming to reduce health disparities. The Compendium compiles in one place descriptions of and links to public datasets and resources that include information about health conditions and other factors that impact the health of minority populations. https://www.minorityhealth.hhs.gov/NPA/Materials/FIHET_Data_Compendium_508_version_FINAL_11_28_2016.pdf
Any of the 59 States, Territories, and Jurisdictions may participate by submitting an application.
The application and annual report will be developed by, or in consultation with, the State maternal and child health agency and comments from any person (including any Federal or other public agency) during its development and after its transmittal.
